Ward: Adult Social Care

Pay Rates: 拢21.80

Shift Patterns: 37.5hrs / M-f / 9-5

Livewell Southwest are currently looking for an experienced Social Worker to join their Adult Social Care Team in Plymouth. If you are looking to develop your NHS experience and provide social care with the support of working within a multidisciplinary team- then please do apply!

Within this role, you will provide a person centred, statutory social work service for a caseload of patients in accordance with relevant policies and procedures. You will undertake complex and proportionate assessments that are evidence-based. You will arrange and review the delivery of social care support to meet individual needs as well as working with partners to safeguard service users.

What you鈥檒l need:

Degree in Social Work or equivalent qualification

HCPC Registration

Experience of conducting assessments identifying health and social care needs

Relevant specialist training in the skills required for the specialty area

In addition to experience, all Qualified roles will be required to provide evidence of:

Registration with the relevant Registration Body if applicable.
