Locum Band 6 Occupational Therapist - The At Home Service

We are seeking a Band 6 Locum Occupational Therapist to join The At Home Service in Pontypridd. This service provides an alternative to hospital admission while supporting earlier discharges. The role involves a mix of clinic sessions and domiciliary visits to assess, plan care, and educate patients to prevent future hospital admissions.

Key Responsibilities:

• Conduct home-based assessments at pace, often responding on the same day
• Plan and implement preventative and admission avoidance interventions
• Deliver and fit equipment to support patient independence and safety
• Work collaboratively with the multidisciplinary team to provide timely and effective care

Requirements:

• HCPC-registered Occupational Therapist with Band 6 experience
• Strong background in hospital avoidance and community-based care
