This impactful role will focus on providing bespoke healthcare pathways and supporting events, childcare initiatives, and family welfare programs.

Key Responsibilities:
- Deliver a fully person-centred Health Support Scheme, liaising with healthcare providers, statutory services, and professionals.
- Build and maintain relationships with healthcare and support networks.
- Provide advocacy for individuals and families navigating healthcare pathways.
- Maintain an active case database, providing regular updates and recording outcomes.
- Assist with the organisation of family and community events, childcare support, and other welfare programs.
- Uphold confidentiality, professionalism, and compliance with GDPR regulations.
About You:
Essential Skills & Experience:
- Proven experience in a welfare, health, or social care setting.
- Excellent written and oral communication skills, with a strong sense of empathy.
- Strong time management and organisational skills, capable of handling multiple projects.
- Competent with Microsoft Office and database software.
- A proactive, conscientious, and independent work ethic.
Desirable Skills & Experience:
- Experience in healthcare or welfare support.
- A good understanding of the challenges individuals and families face.
