Roles and Responsbilities:This role involves assessing individuals who have been discharged from the acute hospital with short-term service needs. It is a dynamic and responsive service requiring comprehensive knowledge of legislation and frameworks, including the Continuing Healthcare (CHC) framework. As an Assistant Team Manager, you will work collaboratively with permanent Assistant Team Managers, supporting the Team Manager by providing day-to-day operational management. Your responsibilities include attending multi-disciplinary team meetings, providing regular supervisions, both monthly and informal, to staff including locums, and ensuring compliance with legislation and policies within expected time frames.What you will do:

- Coordinate and support the day-to-day operations of the team.- Conduct assessments for individuals discharged from acute hospital settings.- Attend multi-disciplinary team (MDT) meetings to ensure integrated care.- Provide supervision and support to team members, including both permanent staff and locums.- Ensure adherence to relevant legislation and policies.Perks and benefits:
