What you will do:

  • Work within the Multi-Agency Safeguarding Hub to coordinate care for vulnerable adults.
  • Assess and implement a range of social care services, ensuring compliance with relevant legal frameworks and standards.
  • Provide supervision and support to newly qualified social workers and Care Navigators within the service.
  • Carry out complex needs assessments and create comprehensive support plans.
  • Collaborate with multi-disciplinary teams and external services for effective signposting and referrals.
  • Engage in safeguarding processes and ensure adherence to best practice and legal requirements.
  •  Conduct Best Interest assessments in relation to Deprivation of Liberties.
  • Effectively communicate sensitive information to stakeholders and collaborate effectively with community forums.
