Randstad are recruiting for a Qualified Social Worker to join Wirral Council's Review Team as a Team Manager. This is a full time role, offering a fantastic pay rate of £56 per hour.

Responsibilities:

  • Sound knowledge of the Care Act 2014, Mental Capacity Act, Safeguarding and Continuing Health Care.
  • Desirable knowledge of the 3 conversation models.
  • Manage a team of five agency social workers.
  • The role is to ensure the five social workers undertake reviews in care homes and in a person's own home in a timely manner.
  • The role will require the manager to allocate the work, monitor throughout, and assure the assessments.
  • Ensure continuing health care checklists are implemented.
  • Cover a wide remit of reviews and reassessment across the Wirral, working with persons recently discharged from hospital and those residing in the community.
  • Support with various adult social care projects and at risk care home and domiciliary providers.
  • Collaborate with Wirral contracts & commissioning and quality improvement teams, adhering to outcomes focused and legal literate practices.
  • Support with the latter service delivery, utilising assessment, and intervention skills, framed in legal literate practices.

Requirements:

  • Social Work England Registered
  • Enhanced DBS
  • Social Work Degree or equivalent
