The successful candidate will work as part of a multidisciplinary team, managing a caseload of elderly adults with a variety of mental health conditions, including dementia, depression, anxiety, and psychosis.Role Responsibilities:

  • Conduct initial assessments and ongoing reviews of patients, ensuring that care plans are regularly updated and reflect the patient's evolving needs.
  • Ensure timely and appropriate interventions are put in place, working collaboratively with other professionals and services
  • Ensure accurate and timely documentation of all medication administration in line with best practice guidelines.
  • Monitor and assess patients for side effects and any adverse reactions to medications.

Essential requirements:

  • Registered Mental Health Nurse (RMN) with an active NMC pin.
