The position is for a Keyworker responsible for managing a caseload of Older Adults with diverse mental health issues and complex needs.
The role requires proficiency in RiO documentation and involves conducting assessments, reviews, and interventions.
Candidates must have experience administering Depot injections and participating in multi-disciplinary reviews both in the community and for inpatients.
The Keyworker will collaborate with various members of the multi-disciplinary team and external agencies to enhance patient care.
The job offers a salary of £35,000 per year and requires at least 2 years of relevant experience.

Monday - Friday
9-5pm

Keyworker to manage caseload for Older Adults with varying mental health issues, co-morbidities and complex presentations. Carrying out assessments, reviews, interventions and completing discharges alongside relevant documentations to support.
Proficient in use of RiO documentations.
Administering Depot injections.
Completing multi-disciplinary reviews in the community and inpatients on Mental Health Act sections.
Liaising with relevant members of the multi-disciplinary team and external agencies to support patient care.
