Expressions of Interest - Service Care Bank Team: Primary Care Pharmacist PrescribersLocation: Blackburn, Preston, Bolton, Blackpool, Manchester Duties:
- Conduct medication reviews, ensuring safe and effective prescribing within a primary care setting.
- Work closely with GPs, nurses, and other healthcare professionals to optimise patient care.
- Manage and monitor medication therapy, providing advice and support to patients as required.
- Provide expert advice on the safe use of medications, including potential side effects and interactions.
- Review patient records and collaborate with other healthcare providers to ensure treatment plans are effective.
- Deliver high-quality clinical services in line with local protocols and regulatory standards.
- Participate in multidisciplinary team meetings and provide input on medication-related issues.
- Assist in the training and development of other healthcare professionals.
Skills & Experience Required:
- Qualified Pharmacist with current GPhC registration.
- Experience with EMIS and S1.
- Independent prescribing qualification is essential.
- Previous experience working in a primary care or clinical setting is highly desirable.
- Strong knowledge of medication management, clinical guidelines, and treatment protocols.
- Ability to work effectively in a team and independently.
- Excellent communication skills, both written and verbal, with the ability to engage with patients and healthcare professionals.
- Strong problem-solving skills with attention to detail.
- Demonstrable commitment to ongoing professional development and learning.
