Expressions of Interest - Service Care Bank Team: Primary Care Pharmacist PrescribersLocation: Blackburn, Preston, Bolton, Blackpool, Manchester Duties:

  • Conduct medication reviews, ensuring safe and effective prescribing within a primary care setting.
  • Work closely with GPs, nurses, and other healthcare professionals to optimise patient care.
  • Manage and monitor medication therapy, providing advice and support to patients as required.
  • Provide expert advice on the safe use of medications, including potential side effects and interactions.
  • Review patient records and collaborate with other healthcare providers to ensure treatment plans are effective.
  • Deliver high-quality clinical services in line with local protocols and regulatory standards.
  • Participate in multidisciplinary team meetings and provide input on medication-related issues.
  • Assist in the training and development of other healthcare professionals.

Skills & Experience Required:

  • Qualified Pharmacist with current GPhC registration.
  • Experience with EMIS and S1.
  • Independent prescribing qualification is essential.
  • Previous experience working in a primary care or clinical setting is highly desirable.
  • Strong knowledge of medication management, clinical guidelines, and treatment protocols.
  • Ability to work effectively in a team and independently.
  • Excellent communication skills, both written and verbal, with the ability to engage with patients and healthcare professionals.
  • Strong problem-solving skills with attention to detail.
  • Demonstrable commitment to ongoing professional development and learning.
