We are excited to announce an opening for a Clinical Pharmacist at £40 per hour (Ltd), offering up to 5 days per week onsite until the end of March 2025, with the potential for an extension once the new budget is confirmed.This opportunity is with a popular Preston client, connected to a group of practices within the Primary Care Network and ran by the PCN director.This is a fantastic opportunity for a dedicated and motivated pharmacist to make a significant impact on patient care. You will work alongside a multidisciplinary team to manage chronic diseases, conduct clinical medication reviews, and optimise medicines.In this role, you will support both patients and colleagues, contributing to high-quality, integrated care through effective collaboration with community and hospital pharmacies.Key Responsibilities:

  • Lead long-term condition clinics and medication reviews.
  • Provide medication support, including repeat prescriptions and acute requests.
  • Drive medicines optimisation and quality improvement initiatives.
  • Collaborate with external healthcare services to improve patient outcomes.

To succeed in this role, we are looking for a pharmacist with:

  • A Master's degree in Pharmacy (MPharm) or equivalent.
  • GPhC registration and a minimum of 2 years post-qualification experience.
  • Strong clinical knowledge, particularly in long-term conditions.
  • Excellent communication skills and a commitment to professional development.
  • General practice experience and knowledge of Emis Web are essential.
  • V300 prescribing qualification.
