Duties within your role

  • Social worker to support with hospital discharge, admission avoidance and discharge to assess in the community. The service provides a multi-disciplinary single point of access to, and support through, community intermediate care services; supporting hospital discharge and preventing hospital/care home admissions . Empathetic, compassionate and resilient, you will ensure people receive the right support at the right time that enables them to achieve their best level of independence and live their life in a way that they choose. Using a strengths-based and positive risk management approach, you will complete high quality assessments and reviews and develop person-centred support plans. Where appropriate, you will work within the Hospital Discharge to Recover, Re-able and Assess model
