JOB PURPOSE

Working as part of the Adult Clinical Services to provide direct and indirect clinical advice, assessments, interventions, treatment, and reviews to support the Physical Health care needs of adults residing within our Learning Disability, Mental Health and Acquired Brain Injury services across England and Wales.

This role reports in to our Associate Clinical Director and this opportunity offers great work-life balance and flexibility. MAIN DUTIES AND RESPONSIBILITIES

Clinical

  • To work autonomously, and as part of a Multi-disciplinary Team, to provide physical health care input to services within the designated region, including providing clinical support and advice to professional colleagues.
  • To develop health Promotion activities within services and show due regard to the impact of health inequalities.
  • To work as part of the Multi-Disciplinary team and with external agencies to ensure safe and effective joined-up working.
  • To formulate and oversee the Physical Health elements of service users holistic care plans, including, but not limited to, advice around Medication, Epilepsy, Diabetes, infection control, pain management and End of Life Care including RESPECT plans.
  • To undertake physical health care assessments and interventions appropriate to the work of the group, drawing on a range of Adult General Nursing evidence-based methods, models and tools as appropriate.
  • To ensure robust risk management and to report all accidents or incidents.
  • To provide Safe, Effective, Responsive, caring and well led clinical support, in line with the regulatory requirements of the CQC or CIW.

Teaching/ Training/ Supervision And Consultation

  • To provide clinical training and advice to care teams and to share skills and learning with internal and external professionals as appropriate.
  • To consult with and advise colleagues from other professions on Adult General Nursing Physical Healthcare assessment and treatment methods.

Policy And Service Development

  • To participate in meetings concerned with the delivery and development of clinical services and contribute a Physical Healthcare perspective as appropriate.
  • Contribute as required to the planning and development of operational policies and service delivery by undertaking agreed-upon project work.

Evaluation/Research/Development

  • To remain informed of and critically evaluate current research to support evidence-based practice in own professional work and to contribute this perspective in the practice and development of the group services.

Professional/Administrative

  • To participate positively in regular management and clinical supervision, and an annual Appraisal.
  • To work within the Code of Ethics and the relevant professional practice guidelines of the Royal College of Nursing and the professional guidance frameworks of the Nursing and Midwifery Council and the group.

Personal Development, Supervision and Training

  • Continuously develop professional knowledge and expertise, attending and participating in learning and development events, meetings, conferences, and events (as requested/approved by the Associate Clinical Director for Adults) to ensure up-to-date knowledge relevant to the role and service development.

ESSENTIAL CRITERIA:

  • Must have a full UK driving license and be happy to travel nationally to the business needs
  • Must have your own vehicle
  • Must have an active NMC pin with no restrictions
