This includes individuals with:
  • Physical disabilities
  • Learning disabilities
  • Neurodiverse conditions
  • Acquired brain injuries
  • Chronic health conditions
  • Older people requiring care and support
In this role, you will conduct strengths-based reviews, ensuring that care and support plans align with individuals’ needs and promote their independence and well-being. You will work collaboratively with individuals, families, and multidisciplinary teams to facilitate positive outcomes.Key Responsibilities:
  • Undertake strengths-based reviews, ensuring care plans remain appropriate and effective.
  • Complete Mental Capacity Act (MCA) assessments, ensuring individuals' rights and best interests are upheld.
  • Carry out Continuing Health Care (CHC) checklists and Decision Support Tools (DSTs) as required.
  • Work collaboratively with partner agencies, care providers, and health professionals.
  • Apply relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005, to practice.
  • Maintain clear and accurate records using the appropriate case management systems.
Requirements:
  • Qualified Social Worker with a minimum of 2 years post-qualification experience.
  • Social Work England (SWE) registration.
  • Strong understanding of strengths-based approaches and person-centered care.
  • Experience in conducting MCA assessments and CHC checklists.
