- Undertake strengths-based reviews, ensuring care plans remain appropriate and effective.
- Complete Mental Capacity Act (MCA) assessments, ensuring individuals' rights and best interests are upheld.
- Carry out Continuing Health Care (CHC) checklists and Decision Support Tools (DSTs) as required.
- Work collaboratively with partner agencies, care providers, and health professionals.
- Apply relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005, to practice.
- Maintain clear and accurate records using the appropriate case management systems.
- Qualified Social Worker with a minimum of 2 years post-qualification experience.
- Social Work England (SWE) registration.
- Strong understanding of strengths-based approaches and person-centered care.
- Experience in conducting MCA assessments and CHC checklists.
