- Conduct comprehensive Care Act and Mental Capacity assessments for individuals in hospital or care home settings.
- Evaluate care plans to ensure services align with individual needs and improve outcomes.
- Liaise with multidisciplinary teams to facilitate seamless hospital discharges and appropriate care placements.
- Review existing care arrangements, ensuring compliance with health and social care policies.
- Work collaboratively with healthcare providers, families, and carers to achieve person-centred outcomes.
- Record accurate case notes and reports in line with organisational standards and statutory guidelines.
- Qualified social worker with current registration with Social Work England.
- Minimum of 2 years’ post-qualification experience in a relevant setting (hospital discharge planning or care home reviews)
- Strong understanding of the Care Act and Mental Capacity Act.
- Experience conducting assessments and reviews in health and social care environments.
- Excellent communication skills to engage with diverse stakeholders, including residents, families, and professionals.
- Ability to work independently in a fast-paced environment while managing competing priorities.
