What you will do:

Provide comprehensive social work and care management services to residents of Hounslow and their carers.
Conduct social care assessments to identify needs and desired outcomes, ensuring residents are central to all interventions while promoting independence through choice and control over service delivery.
Protect the rights and promote the interests of residents and their carers, ensuring they are safeguarded in accordance with both the Council’s and Pan London Safeguarding Policy.
Engage in safeguarding investigations, contribute to strategy meetings, and implement necessary actions.
Collaborate with a multidisciplinary team to deliver holistic, person-centred care and support.
