Are you a dedicated Band 6 Community Psychiatric Nurse (CPN) or Social Worker looking to make a meaningful difference? A great client of ours is seeking a professional for an Adult Community Mental Health Team, supporting individuals aged 18 to 65 in the Reading area. They offer a flexible work environment with a balance of on-site and remote working.
Role OverviewYou will be responsible for managing a caseload of up to 30 service users. You’ll play a vital role in care coordination, delivering high-quality support and interventions tailored to each individual’s needs.
Key Responsibilities- Manage a caseload of up to 30 people aged 18–65 with diverse mental health needs
- Provide care coordination and case management within the community
- Deliver person-centered assessments, interventions, and care plans
- Work collaboratively with other healthcare and social care professionals
- Attend team meetings and case discussions
- Engage with clients and their families to promote recovery and wellbeing
- Valid NMC registration for CPN or SWE registration for Social Workers, with no restrictions
- Experience in community mental health, ideally in an adult setting
- Ability to manage a caseload efficiently, maintaining a high standard of care
- A car and valid driver’s license to travel within the community
- Strong interpersonal and communication skills
- Competitive hourly rate (£25–£27 per hour)
