  • Liaise with MDT members in treatment and management care plans and working closely
  • Support OT /PT with the recommendations for patient centred approach to achieve their rehab goals
  • Create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required.

What you’ll need:

  • Diploma / Degree in Physiotherapy / Occupational therapy.
  • HCPC Registration
  • 2-3 years UK NHS Experience
  • should be able to drive and own a car for work purposes with business insurance
