Job Title: Social Worker - Community Mental Health Team (CMHT)
We are looking for a skilled and compassionate Social Worker to join our Community Mental Health Team (CMHT). In this role, you will conduct assessments, develop care plans, and provide tailored support to individuals with mental health needs, working closely with multidisciplinary teams and external agencies. The ideal candidate will have a Social Work degree, registration with Social Work England, and a strong understanding of mental health legislation.
