Job Summary:
We are seeking a compassionate and dedicated Access to Work Support Worker to join our team. The ideal candidate will work with individuals who have disabilities or long-term health conditions, helping them access and thrive in their workplace or educational environment.
Key Responsibilities:
- Support Provision:
- Provide practical support to individuals in the workplace or education settings, in line with Access to Work guidelines.
- Ensure the individual's needs are met, supporting their independence, productivity, and well-being.
- Assessment and Liaison:
- Conduct assessments of individual needs in collaboration with the person requiring support.
- Review and adjust support plans as required, to ensure they meet the individual's evolving needs.
- Documentation and Reporting:
- Maintain accurate records of support provided, progress, and outcomes.
- Confidentiality and Data Protection:
- Adhere to all confidentiality and data protection policies when managing sensitive information regarding individuals and their circumstances.
- Training and Guidance:
- Assist individuals in learning how to use assistive technologies or any other tools necessary to support their role.
- Ongoing Support:
- Monitor and assess the effectiveness of the support plan on an ongoing basis, adjusting as necessary.
- Provide emotional and practical support to help the individual navigate any challenges that arise in their role.
Person Specification:
- Previous experience of working with individuals with disabilities or long-term health conditions.
- Good communication skills, with the ability to interact with a diverse range of individuals.
- Ability to assess needs and tailor support to the individual.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Confidentiality and sensitivity when working with personal and confidential information.
- Empathy and patience, with a commitment to supporting individuals to reach their potential.
- Experience in assistive technology or knowledge of workplace adjustments.
- Knowledge of relevant health and safety regulations and their application in workplace settings.
Working Conditions:
- Flexible working hours may be available depending on the needs of the individuals supported.
- Occasional travel between work sites.
- Some remote working options may be available.
