- Provide expert wound and medication management in the community
- Deliver catheter care and support for patients with chronic conditions
- Offer compassionate palliative and end-of-life care
- Work collaboratively with a multidisciplinary team to ensure patient-focused care
- Assess, plan, and evaluate care tailored to individual patient needs
- Registered Nurse (RGN) with a valid NMC pin
- Proven experience in community nursing (essential)
- Strong clinical expertise in wound care, catheter care, and palliative support
- Able to work independently and manage a diverse caseload
- Full UK driving licence and access to a vehicle (preferred)
