Responsibilities:
- Undertake holistic assessments and develop transition plans that will inform the appropriate pathway and support for the young person
- Co work with children's services and discharging the care act duties
To be considered for the Social Worker role:
- 1+ years experience of working within a similar social work setting
- Diploma/Degree in Social Work
- Registration with Social Work England
- Evidence of working with a complex caseload
- Experience of application of the Care Act 2014
