Job Overview
We are recruiting an Outreach Worker to join our client's Hillingdon Outreach Team. This is an opportunity to make a significant impact on the lives of rough sleepers by assessing their needs, advising them on available options, and linking them with external services. The role primarily covers the Heathrow area and is both challenging and rewarding.
Key Responsibilities
- Work in pairs for outreach shifts and independently with risk-assessed service users to attend community appointments.
- Support rough sleepers in accessing appropriate social care services and accommodation.
- Act as the first response to referrals, conducting needs and risk assessments for new rough sleepers.
- Update assessments for rough sleepers already known to the team.
- Maintain accurate records and update the CHAIN database.
- Determine the best course of action for each client's situation.
- Collaborate with external agencies, including local authorities, health services, housing providers, police, and drug & alcohol services.
Requirements
- Excellent interpersonal skills.
- Flexibility and responsiveness to meet the needs of rough sleepers.
- Confidence and ability to manage a caseload of complex clients.
- Strong IT skills to collect, collate, and interpret data.
- Ability to represent the client at external meetings and liaise effectively with outside agencies.
- DBS Check: An enhanced adult and child DBS check is required. If you need one, please get in touch.
Ideal Candidate
- Strong communication skills and the ability to build effective working relationships.
- Ability to empower individuals to make positive changes.
- Willingness to work flexibly on a shift rota basis, engaging with rough sleepers in various locations within the borough.
- Experience working with homeless individuals in a social care setting (direct outreach experience is desirable but not essential).
