This is a fantastic opportunity for someone experienced with routine Practice Nurse duties, as well as experience with Chronic Disease Management.
Salary – £35,000 - £42,500 per annum DOE FTE + NHS pension + 6 week annual leave + Bank Holidays

Your role –
- Essential experience as a Practice Nurse comfortable with all routine Practice Nurse responsibilities and Chronic Disease Management experience
- Opportunity to progress and become a prescriber if not already
- Flexibility around start and finish times
- Benefit from a very supportive MDT around you
- No home visits
- 30-37.5 hours per week
