What You'll Be Doing:

Join our Primary Care Mental Health Liaison Team, where you will:

• Provide specialist mental health support within a primary care setting• Conduct comprehensive assessments and risk management for service users• Deliver brief interventions and signposting to appropriate services• Work closely with GPs, multidisciplinary teams, and community services• Support early intervention and prevent hospital admissions

Requirements & Qualifications:

To be successful in this role, you will need:

• NMC or HCPC registration (RMN, Social Worker, or Occupational Therapist)• Experience in primary care mental health, crisis intervention, or CMHT• Strong assessment, risk management, and care planning skills• The ability to work autonomously and within a multidisciplinary team• A passion for improving access to mental health support in primary care

Working Hours & Contract Details:

This is a Monday to Friday, 9 AM - 5 PM role with an ASAP start.
