  • Deliver person-centred care and support tailored to individual goals and preferences.
  • Take a holistic approach, supporting families and carers to enhance the lives of those in their care.

Experience:
  • Post-qualification experience in social care, health, or related fields.
  • Expertise in risk management, safeguarding enquiries, and supporting individuals with medium to high-level complex needs.
  • Proven ability to work collaboratively in multi-agency environments.
Skills and Abilities:
  • Excellent communication, mediation, and negotiation skills.
  • Strong IT and administrative capabilities.
  • The ability to manage workloads independently and as part of a team.
  • A commitment to promoting independence and innovative approaches to care.
  • Flexibility to travel and work varied hours to meet service needs.
Knowledge:
