What you will do:

  • Assess the needs, plan, and deliver care from initial assessment to discharge from services, ensuring the best outcomes for service users.
  • Collaboratively implement and evaluate personalised care plans, working alongside service users, carers and other professionals.
  • Empower and support service users on their recovery journey, helping them regain independence.
  • Work within community settings, advocating for individuals and providing therapeutic interventions as required.
  • Supervise nonqualified staff and ensure high standards of care are maintained.
  • Engage closely with external partners including community services, social services, and other healthcare agencies to provide comprehensive support.
  • Provide essential signposting to additional services to enhance holistic care.
  • Utilise a basic understanding of the Mental Health Act to inform practice.
