Job Purpose
As a Wellbeing Key Worker, you will deliver specialised guidance and training services to individuals transitioning back into the community. You will play a crucial role in supporting these individuals by creating personalised wellbeing plans, facilitating progress toward health and employability goals, and building strong connections with local support services. Your impact will not only affect their personal development but will also contribute to safer communities through effective rehabilitation.

Key Responsibilities

  • Provide tailored Wellbeing support that meets the unique needs of each service user.
  • Manage a caseload, guiding each individual toward positive personal outcomes and progress.
  • Conduct comprehensive assessments, including screenings for learning difficulties and disabilities.
  • Develop strong partnerships with local health and community services for seamless referrals.
  • Use a range of channels—digital platforms, workshops, and face-to-face sessions—to deliver support.
  • Produce high-quality action plans that empower individuals to achieve their personal and professional goals.
  • Plan, lead, and evaluate group workshops that enhance wellbeing and employability skills.
  • Ensure compliance with Wellbeing contract requirements, proactively participating in self-assessment and continuous improvement.

About You
You’ll thrive in this role if you’re a compassionate and dedicated individual, skilled in providing personalised support within a target-driven environment. You will be able to communicate effectively with service users, team members, and community partners, and feel confident delivering support through various channels.

Essential Skills and Experience

  • Strong interpersonal skills with a caring, sensitive approach to support.
  • Ability to meet targets and deliver one-on-one and group sessions effectively.
  • Digital proficiency to deliver services through online platforms.
  • Background in supporting personal wellbeing and progression, ideally within a Criminal Justice setting.
  • Recent experience managing a caseload and creating action plans to support service users in reaching their wellbeing and employment goals.

Desirable Qualifications

  • Substance Misuse Awareness Level 3, Counselling Skills for Health Professionals, or ACE Training.
  • Mental Health First Aid and Suicide Awareness/Prevention.
  • Knowledge of benefits systems and experience with behavioural change tools (e.g., ITEP/Node-Link mapping).
