Community Psychiatric Nurse - Older Adults (Dementia Care) - Band 6

Are you experienced in Dementia Care and seeking a new challenge? 

We are looking for a skilled Band 6 Community Psychiatric Nurse to join our Assessment and Diagnosis Pathway for Dementia Care. This temporary role offers an excellent opportunity to contribute to the care of those living with dementia in a supportive, collaborative environment.

Role Summary

As part of our team, you will work closely with the Operational Lead and Nurse Practitioners to manage referrals coming into the service. Your role will include:

  • Reviewing referral information and gathering any necessary additional details to facilitate timely assessments.
  • Collaborating with primary care teams, other services, patients, and their families to ensure a holistic approach to care.
  • Supporting the assessment and diagnosis process by ensuring appropriate information is collected and shared with relevant stakeholders.
Ideal Candidate
  • Experience in Dementia Care or working in an Older Adult setting is essential.
  • Strong communication and organizational skills with the ability to work autonomously and within a team.
  • Ability to build strong relationships with both patients and other healthcare professionals.
Working Arrangements

This role allows for a blended approach: the majority of work can be done from home, with some office time required, particularly during the induction and training phase.
