Our client, Shropshire County Council, is seeking social workers for its Hospital Discharge team at Princess Royal Hospital and Royal Shrewsbury Hospital.
Pay rate available is up to £38 umbrella.

Details of the role involve:
- The primary goal is to provide the best service for individuals who require support with complex discharge and admission avoidance from the Emergency Department and Frailty Assessment Unit under the principles of the Care Act (2014).
- The model we support aims to get people home as soon as possible to improve length of stay, enhance flow in the hospital and as a system, support the hospital to have the best performance and quality for the people who need acute intervention.
- Royal Shrewsbury Hospital Guildhall and Shrewsbury Princess Royal Hospital
Requirements of the role:
- Qualified Social Worker
- Fully Social Work England registered
- Must have significant experience in adult social care
- Ideal candidate will have expert knowledge of working in a Hospital Discharge setting
