Social Worker - Hospital Discharge Team

We are seeking a dedicated Social Worker to join Shropshire Council's Hospital Discharge team, based at Princess Royal Hospital and Royal Shrewsbury Hospital. This role offers £38 per hour.

About the Role:

The primary goal is to provide the best service for individuals who require support with complex discharge and admission avoidance from the Emergency Department and Frailty Assessment Unit, under the principles of the Care Act (2014). This model aims to facilitate timely discharge to improve hospital flow and performance.

Key Responsibilities:
  • Provide comprehensive social work support to patients requiring assistance with hospital discharge.
  • Conduct assessments to determine individual needs and develop appropriate care plans.
  • Work collaboratively with medical staff, patients, and families to ensure smooth and safe transitions from hospital to home or other care settings.
  • Navigate complex discharge processes, addressing issues related to accommodation, finances, and ongoing care.
  • Support admission avoidance initiatives by providing timely interventions and assessments in the Emergency Department and Frailty Assessment Unit.
  • Adhere to the principles of the Care Act (2014) and promote person-centered care.
