About the Role:We are seeking an Business Coordinator to manage referrals through the D2A (Discharge to Assess) pathway, ensuring smooth transitions from hospital to Reablement services. You will coordinate the hospital hub inbox, maintain accurate data, and liaise with health and social care professionals to ensure safe and effective discharges.

Key Responsibilities:

  • Coordinate referrals and ensure appropriate discharge information for OTs and Social Workers.
  • Collaborate with health and social care professionals for service user well-being.
  • Maintain accurate data using EPIC and LAS systems.
  • Cover the HSWT inbox and support case transfers to Reablement.
  • Assist the hospital manager with additional tasks as needed.

Person Specification:

  • Experience in adult social care and hospital discharge systems.
  • Knowledge of Occupational Therapy risk assessments and physical disabilities.
  • Strong communication, IT, and time management skills.
  • NVQ Level 3 in Care or equivalent, with a commitment to further qualifications.
