About the Role

This exciting position focuses on assessment, care planning, and service purchasing, all within a dynamic, multi-disciplinary team.
What You'll Do
- Manage a varied caseload, delivering high-quality social work within the framework of relevant legislation.
- Provide expert advice, advocacy, and therapeutic interventions to clients and their carers.
- Collaborate effectively with colleagues across the multi-disciplinary team to coordinate care and ensure excellent outcomes for service users.
- Promote independence and wellbeing through person-centred planning and tailored support services.
