Key Responsibilities
  • Case Management: Handle assigned cases by developing outcome-oriented plans to address the specific needs and risks for children and their families.
  • Relationship Building: Establish strong connections with children, young people, and families to enhance support impact. Ensure written plans accurately reflect the views of those you support.
  • Service Audits: Participate in routine audits to maintain high standards in social work practices and documentation, adhering to the Quality Assurance Framework.
  • Financial Assessments: Perform financial assessments as needed and make informed decisions on contributions and levels of service.
  • Collaboration with Partners: Build effective partnerships with community organizations, schools, police, healthcare providers, and other local resources.
  • Record Keeping: Maintain comprehensive, accurate, and up-to-date case records using the Integrated Children’s System.
  • Case Transitioning: Manage case transitions within other service areas as required, ensuring a clear and documented transfer process.
  • Team Support: Offer support to team members and contribute to projects or duties relevant to the role.
Requirements
  • Qualification: A recognized Social Work qualification or equivalent.
  • Knowledge: Solid understanding of social work legislation.
  • Registration: Registered or eligible for registration with the appropriate regulatory body.
