Key Responsibilities:


On-Call Duties:

  • Participate in an on-call rotation to provide emergency care coverage after hours.
  • Deliver high-quality, compassionate care directly to clients in their homes during on-call shifts.


Client Care:

  • Assist clients with personal care, medication management, and daily living activities.
  • Conduct spot checks and risk assessments to ensure clients needs are met and documented, maintaining the quality of care.
  • Monitor clients health and well-being, promptly reporting any changes to the Deputy Care Manager.


Team Supervision:

  • Lead and motivate a team of care assistants, ensuring exceptional standards of care delivery.
  • Provide ongoing training, mentorship, and support to team members to foster their professional development.


Record Keeping:

  • Maintain meticulous and accurate records of client carecompleting care plans and documenting updates in the Bright Dawn Home Care system.
  • Ensure all care assistants are informed and compliant with any changes to client care plans or schedules.


Scheduling:

  • Collaborate with the Deputy Care Manager to develop and manage staff rosters.
  • Communicate any unforeseen changes in schedules to clients promptly and professionally.


Communication:

  • Build strong relationships with clients, families, and your care team to guarantee seamless continuity of care.
  • Serve as a point of contact for addressing client concerns and feedback.


Additional Responsibilities:

  • Assist the Deputy Care Manager with covering uncovered shifts as necessary.
  • Support the development and implementation of care plans that meet the unique needs of each client.


Requirements:

  • Valid UK driving license and access to your own vehicle.
  • Proven experience in a similar role (such as care coordination, team leader, or senior care assistant).
  • Exceptional communication and interpersonal skills.
  • Strong organizational skills with meticulous attention to detail.
  • Ability to work independently and make decisions with minimal supervision.


Preferred Qualifications:

  • Experience in a domiciliary care environment.
  • Previous supervisory experience in a healthcare setting.
  • Relevant certification in care coordination or a related field.
  • Proficiency in electronic record-keeping systems.
