Job Title: Live Well Coordinator

Location: Croydon - Hybrid working, 3 days office / 2 days remote working - CR0 1EA, including travel to other locations around the borough.

Hourly rate 22.55 PAYE / 29.25 UMB Per hour

Contract Length: 3-month temporary assignment (possibility of extension)

Working Pattern: Full Time, Monday - Friday, 36 hours

About Us:

An integrated Adult Healthy Lifestyle service, Live Well Croydon, is dedicated to increasing life expectancy, improving health, and reducing health inequalities through accessible, evidence-based lifestyle interventions. We engage communities using a resident-centred approach to empower individuals to identify their own solutions and co-design provision.

Role Purpose:

As the Live Well Croydon Coordinator, you will assist the Live Well Croydon Service Manager in delivering the Live Well Croydon service. Your responsibilities will include managing referrals, client communications, team calendars, identifying key venues for delivery, engaging with and training internal and external staff, and ensuring the quality of service provision and delivery. This role may require some evening and weekend working.

Key Responsibilities:

  • Manage and triage client referrals.
  • Oversee the outreach plan, including automated communications.
  • Train internal and external frontline staff.
  • Provide expert advisory services in lifestyle interventions.
  • Ensure the quality and performance of interventions across various partners.
  • Manage the client management system and ensure accurate data input and reporting.
  • Maintain confidentiality and adhere to data protection regulations.
  • Contribute to the continuous improvement of service provision.

Essential Skills and Experience:

  • Knowledge of behaviour change interventions and healthy lifestyle services (desirable).
  • Comfortable and confident with telephone conversations.
  • Ability to work collaboratively and manage relationships across departments.
  • Strong communication skills, both verbal and written.
  • Experience in managing referrals and delivering high-standard customer service.
  • Proficiency in IT, including PowerPoint, client management databases, Outlook, and Excel.

Special Conditions:

  • Travel between sites may be necessary.
