- Carry out holistic occupational therapy assessments and deliver tailored interventions
- Work collaboratively with service users, families, carers, and the wider MDT
- Promote recovery, independence, and social inclusion in community settings
- Support service users with goal setting, vocational activities, daily living skills, and social participation
- Contribute to risk assessments, care plans, and CPA reviews
- Maintain accurate clinical records in line with Trust policies
- Participate in team meetings and clinical supervision
- Liaise with community services, primary care, and voluntary sector partners
- HCPC registered Occupational Therapist
- Experience working at Band 6 level within community mental health settings
- Strong understanding of recovery-focused and trauma-informed practice
- Able to manage a caseload independently
- Excellent communication and organisational skills
- Right to work in the UK
