Band 6 Agency Social Worker - Acute Mental Health Service (Discharge Team)

About the Role:
West London Mental Health Trust is seeking a dedicated and experienced Band 6 Social Worker to join our Discharge Team within the Acute Mental Health Service on a one-year fixed-term contract. This exciting opportunity is perfect for a social worker with a strong background in mental health and discharge planning, who is ready to work in a fast-paced, rewarding environment.
As part of the Discharge Team, you will play a key role in facilitating the smooth and timely discharge of patients from the acute setting to appropriate community care, ensuring that individuals receive the support they need to transition successfully.
Key Responsibilities:
  • Undertake social work assessments and create discharge plans for service users.
  • Work collaboratively with multi-disciplinary teams to ensure holistic care for individuals.
  • Provide effective discharge planning and risk management.
  • Support service users in accessing community services and resources post-discharge.
  • Maintain high standards of record-keeping and report writing.
  • Offer expert advice and support to colleagues within the team.
Person Specification:
  • Qualified Social Worker with HCPC registration.
  • Experience in mental health services, ideally in an acute setting.
  • Knowledge of discharge planning, safeguarding, and risk management.
  • Ability to work in a fast-paced and dynamic team environment.
  • Strong communication and organisational skills.
