As a Nurse Case Manager, you will:
  • Manage a caseload of clients, acting as the main point of contact for clinical updates.
  • Develop, implement, and review care plans and risk assessments.
  • Conduct regular reviews of client care plans and documentation.
  • Ensure safe management and administration of medication.
  • Safeguard children and vulnerable adults, escalating concerns as necessary.
  • Monitor, support, and coach care staff, providing on-call support as needed.
  • Promote best practices and provide training to enhance care quality.
  • Liaise with external professionals and multi-disciplinary teams.
