As a Nurse Case Manager, you will:
- Manage a caseload of clients, acting as the main point of contact for clinical updates.
- Develop, implement, and review care plans and risk assessments.
- Conduct regular reviews of client care plans and documentation.
- Ensure safe management and administration of medication.
- Safeguard children and vulnerable adults, escalating concerns as necessary.
- Monitor, support, and coach care staff, providing on-call support as needed.
- Promote best practices and provide training to enhance care quality.
- Liaise with external professionals and multi-disciplinary teams.

The ideal Nurse Case Manager will:
- Hold active registration with the Nursing & Midwifery Council.
- Have experience in clinical training and competency assessments.

- Possess a Full UK Driving Licence and access to a vehicle.
- Be solution-focused and thrive in a fast-paced environment.
