Keystone Care is seeking a dedicated CarePlan Manager to join a new and growing residential healthcare provider in South England. The successful candidate will be responsible for developing, implementing, and managing personalised care plans for our clients to ensure they receive the highest standard of care. The role requires strong organizational skills, excellent communication abilities, and a compassionate approach to care.

Duties and responsibilities:

  • Managed and develop high quality services for all our residents, alongside developing and guiding the staff team
  • Assessing clients' needs and developing tailored care plans
  • Coordinating with healthcare professionals, clients, and their families
  • Monitoring and evaluating the effectiveness of care plans
  • Making necessary adjustments to care plans as clients' needs change
  • Ensuring compliance with health and safety regulations and standards
  • Documenting and maintaining accurate client records
  • Providing support and guidance to care staff

Skills and experience:

  • Strong understanding of healthcare regulations and standards
  • Experience writing and reviewing care plans in a nursing or residential setting
