We are currently seeking a highly organized and compassionate Care Coordinator to join our busy domiciliary care branch in Southampton. As a Care Coordinator, you will play a vital role in ensuring the smooth and efficient delivery of high-quality care services to our clients. The ideal candidate will have excellent communication skills, strong attention to detail, and a passion for providing exceptional care to individuals in need.

Key Responsibilities:

  • Coordinate and schedule care services for clients based on their individual needs and preferences
  • Liaise with clients, their families, and care staff to ensure the delivery of personalized and effective care plans
  • Monitor and review care plans to ensure they are up-to-date and meeting the changing needs of clients
  • Conduct regular assessments and risk assessments for new and existing clients
  • Maintain accurate and up-to-date records in accordance with company policies and procedures
  • Provide support and guidance to care staff, ensuring they have the necessary training and resources to deliver high-quality care
  • Participate in on-call duties on a rotational basis to provide support and guidance to clients and care staff outside of office hours
  • Collaborate with external healthcare professionals and agencies to ensure the best possible care for clients
  • Assist with recruitment and training of new care staff as needed
  • Requirements:

  • Previous experience in a care coordination or similar role within the healthcare industry
  • NVQ Level 3 in Health and Social Care or equivalent qualification
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work independently and as part of a team
  • Proficient in Microsoft Office and other relevant software
  • Knowledge of relevant regulations and legislation in the healthcare sector
  • Compassionate and empathetic nature
  • Full UK driving license and access to a vehicle
