We are currently seeking a highly organized and compassionate Care Coordinator to join our busy domiciliary care branch in Southampton. As a Care Coordinator, you will play a vital role in ensuring the smooth and efficient delivery of high-quality care services to our clients. The ideal candidate will have excellent communication skills, strong attention to detail, and a passion for providing exceptional care to individuals in need.
Key Responsibilities:
Coordinate and schedule care services for clients based on their individual needs and preferencesLiaise with clients, their families, and care staff to ensure the delivery of personalized and effective care plansMonitor and review care plans to ensure they are up-to-date and meeting the changing needs of clientsConduct regular assessments and risk assessments for new and existing clientsMaintain accurate and up-to-date records in accordance with company policies and proceduresProvide support and guidance to care staff, ensuring they have the necessary training and resources to deliver high-quality careParticipate in on-call duties on a rotational basis to provide support and guidance to clients and care staff outside of office hoursCollaborate with external healthcare professionals and agencies to ensure the best possible care for clientsAssist with recruitment and training of new care staff as neededRequirements:
Previous experience in a care coordination or similar role within the healthcare industryNVQ Level 3 in Health and Social Care or equivalent qualificationExcellent communication and interpersonal skillsStrong organizational and time-management skillsAbility to work independently and as part of a teamProficient in Microsoft Office and other relevant softwareKnowledge of relevant regulations and legislation in the healthcare sectorCompassionate and empathetic natureFull UK driving license and access to a vehicleWe offer a competitive salary and opportunities for career development within a supportive and dynamic team.
