Join our Adults Social Work Prevention Team and play a vital role in delivering person-centered support and care to individuals in the community.
As a member of the Adults Social Work Prevention Team, you will work collaboratively with individuals, their families, and relevant stakeholders to assess needs, develop personalized care plans, and coordinate support services to promote independence and quality of life.
Key Responsibilities:
- Conduct comprehensive assessments and reviews under the Care Act 2014, focusing on strengths, assets, and individual goals to maximize independence and well-being.
- Work closely with individuals, families, and community resources to identify appropriate support services and interventions, including minor adaptations, routine equipment, and community-based activities.
- Champion a strengths-based, asset-based approach to assessment, care planning, and review, empowering individuals to take control of their outcomes and goals.
- Support individuals with specialist needs to access personalized care and support services, collaborating with health and social care colleagues to ensure holistic care provision.
- Manage complex moving and handling situations, providing guidance and support to locality team colleagues and homecare providers.
- Engage with internal and external stakeholders, including voluntary, public, and private sector organizations, to develop collaborative partnerships and enhance service delivery.
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes, ensuring compliance with relevant legislation and policies.
Requirements:
- Qualified Social Worker with relevant degree or diploma in social work.
- Registered with the appropriate regulatory body (e.g., Social Work England).
- Experience applying the Care Act 2014, Mental Capacity Act 2005, and managing adult safeguarding inquiries (direct experience preferred but not essential).
