Role Overview:

We are seeking a highly organized and compassionate Care Coordinator to join our team. The ideal candidate will have excellent communication and time-management skills, along with a strong understanding of the care industry. As a Care Coordinator, you will play a key role in coordinating and managing the care services for our clients, ensuring that their needs are met and their care plans are delivered effectively.

Key Responsibilities:

  • Coordinate and manage the care services for our clients, ensuring that their needs are met and their care plans are delivered effectively
  • Liaise with clients, their families, and care staff to ensure that all care needs are met and any concerns are addressed promptly
  • Conduct assessments and develop personalized care plans for clients, taking into consideration their individual needs and preferences
  • Monitor and review care plans regularly to ensure they are up-to-date and reflective of the client's changing needs
  • Schedule and coordinate care staff rotas to ensure adequate coverage and continuity of care for clients
  • Provide support and guidance to care staff, ensuring they have the necessary training and resources to deliver high-quality care
  • Maintain accurate and up-to-date client records and ensure confidentiality is maintained at all times
  • Participate in on-call duties as required, providing support and guidance to care staff and clients outside of regular office hours
  • Requirements:

  • Minimum of 2 years of experience in a similar role, preferably within a domiciliary care setting
  • NVQ Level 3 in Health and Social Care or equivalent qualification
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Knowledge of care industry regulations and standards
  • Ability to work well under pressure and handle multiple tasks simultaneously
  • Proficient in using computer systems and software, including Microsoft Office
