Role Overview:
We are seeking a highly organized and compassionate Care Coordinator to join our team. The ideal candidate will have excellent communication and time-management skills, along with a strong understanding of the care industry. As a Care Coordinator, you will play a key role in coordinating and managing the care services for our clients, ensuring that their needs are met and their care plans are delivered effectively.
Key Responsibilities:
Coordinate and manage the care services for our clients, ensuring that their needs are met and their care plans are delivered effectivelyLiaise with clients, their families, and care staff to ensure that all care needs are met and any concerns are addressed promptlyConduct assessments and develop personalized care plans for clients, taking into consideration their individual needs and preferencesMonitor and review care plans regularly to ensure they are up-to-date and reflective of the client's changing needsSchedule and coordinate care staff rotas to ensure adequate coverage and continuity of care for clientsProvide support and guidance to care staff, ensuring they have the necessary training and resources to deliver high-quality careMaintain accurate and up-to-date client records and ensure confidentiality is maintained at all timesParticipate in on-call duties as required, providing support and guidance to care staff and clients outside of regular office hoursRequirements:
Minimum of 2 years of experience in a similar role, preferably within a domiciliary care settingNVQ Level 3 in Health and Social Care or equivalent qualificationExcellent communication and interpersonal skillsStrong organizational and time-management skillsKnowledge of care industry regulations and standardsAbility to work well under pressure and handle multiple tasks simultaneouslyProficient in using computer systems and software, including Microsoft OfficeCompassionate and empathetic nature with a genuine desire to make a positive impact on people's livesIf you are a dedicated and compassionate individual with a passion for providing high-quality care services, we would love to hear from you.
