Are you passionate about providing care in the community? Do you have access to your own vehicle? Would you like to be part of a dedicated, professional, and friendly team committed to delivering excellent patient care?

We are currently hiring for an exciting community-based role in Hertfordshire. As the team responsible for new patient referrals continues to grow, we’re looking for skilled Health Care Professionals to support a thriving palliative care project. This is an amazing opportunity to make a meaningful impact on the lives of patients and their families.

About the Role:

We’re recruiting experienced Health Care Professionals to provide essential end-of-life care in the comfort of patients' homes. This will include personal care, medication prompting, and offering both physical and emotional support to patients at a critical time in their lives.

The project covers homes across Hertfordshire and parts of Buckinghamshire, including the following locations (please note: you must be willing to travel to any of the areas listed below):

  • Rickmansworth
  • St Albans
  • Harpenden
  • Hemel Hempstead
  • Hatfield
  • Tring

Available Positions:

These roles are available on both a part-time and full-time basis, with a variety of shift patterns to suit your schedule:

  • 08:00-12:00 (Early shift)
  • 14:00-20:00 (Late shift)
  • 08:00-20:00 (Long day shift)

We have flexible hours available from Monday to Sunday, and with our growing project, there are plenty of shifts to choose from.

What We’re Looking For:

  • Access to your own vehicle
  • Manual Handling and Life Support training (we can help arrange this if needed)
  • A current DBS or willingness to obtain one
  • Reliability, punctuality, and a compassionate approach to care
