Qualified Social Worker - Adults with disabilities

We are looking to recruit a Qualified Social Worker - Adults with disabilities within Hertfordshire Council

Responsibilities include:

• Care Act Assessments
• Support Planning and Reviews
• CHC Assessments
• Asylum/NRPF, Transition, Safeguarding
• Undertaking comprehensive social care assessments and reports, with detailed support plans, as appropriate.
• Knowledge and understanding of Direct Payments using the strength-based approach
• Social Worker should have sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools.
• Social worker will also need to communicate closely and sensitively with family members of service users.
  
  
  Work arrangements: Flexible arrangements to include Face to face visits/assessments, Work from Home, and office as required.

Experience preferred:

• Post qualifying experience of working in a Qualified Social Worker - Older People position within the UK Social Care industry.

Minimum Requirements:

• SWE Registration
• 1 year post qualifying experience within the UK Social Care industry
• Enhanced DBS/CRB (we can obtain for you)
• Eligibility to work in the UK
