This role offers an exciting opportunity to work collaboratively in a specialist multi-disciplinary team (MDT), providing efficient, patient-focused care to promote recovery and support discharge planning. You will play a pivotal role in delivering high-quality therapeutic management to patients within our frailty pathway.

Main duties of the job

  • Conduct advanced holistic triage and assessment for patients with complex physical, emotional, and psychological symptoms.
  • Develop and deliver individualised treatment programmes.
  • Act as an advocate for patients and carers, promoting their needs within the MDT.
  • Use evidence-based practice to ensure high standards of care.
  • Provide supervision and support to junior staff, students, and MDT colleagues.
  • Participate in risk assessment and management, adhering to Trust and National protocols.

To be successful in applying for this role you will need:

  • Current relevant professional registration (HCPC Registration)
  • Degree or equivalent in Physiotherapy
  • Previous Band 5 Physiotherapy experience, with exposure to complex symptom management and long-term health conditions.
