Seven Resourcing are currently recruiting for several Support Workers to start a new role in Stockport.

In this role, you will work as a support worker to work within the End-of-Life service to meet the increasing demands for end of life and palliative care.

You will work in partnership with our existing team to deliver person centred care, emotional support to family and uphold the core values of both REaCH and Stockport Councils Adult Social Care services to ensure our service users final wishes to remain at home are met.

This is an interim role initially until March 2025 with the possibility of extension, paying £16.13 an hour.

Responsibilities:

  • To work within the multi-disciplinary integrated care teams that deliver community-based services to a GP Practice population, undertaking nursing, therapy and social care tasks within agreed parameters, delegated by the registered practitioners, as set out in a care plan.
  • To undertake initial visit assessment and support people to create wellbeing / Goals of Care plans which are personalised to them.
  • To support people who may have complex health or social care issues or challenging behaviour, such as dementia, mental health issues, drug and alcohol issues, and people on the End-of-Life pathway, following an assessment of risk being undertaken by the registered professional. To highlight any further risks / concerns that you identify to the registered professional and discuss these with your line manager

Applicant Requirements:

  • Experience: Experience of working with older people in their own homes.
  • Qualifications: NVQ Care/Promoting Independence (Level 3) (desirable not essential).
  • Compliance: Enhanced DBS, Right to Work in the UK
  • Hours: various hours per week, Monday to Sunday
  • Travel: Full UK driving license (essential)
