Social Worker - Reablement Team - Stockport Metropolitan Borough Council - up to £30 per hour.

Stockport Metropolitan Borough Council is seeking a skilled and enthusiastic Social Worker to join our Reablement Team. We are committed to supporting individuals to regain their independence and live fulfilling lives, and we need talented professionals like you to help us achieve this.

About the Role:

As a Social Worker within our Reablement Team, you will play a crucial role in supporting the REaCH service to meet service demands. You will achieve this by completing reviews, Care Act assessments, and Mental Capacity assessments. Critically, you will progress individuals with eligible Care Act needs onto further long-term commissioned services/packages of care. This rewarding role requires strong assessment skills, excellent communication abilities, and a commitment to person-centred practice.

Key Responsibilities:

  • Work within a reablement team/service to monitor service users' progress and assess any ongoing needs.
  • Hold a short-term caseload, monitoring, prioritizing, and reviewing cases effectively.
  • Conduct assessments under the Care Act 2014, including formal mental capacity assessments when required.
  • Respond to statutory and non-statutory safeguarding concerns.
  • Provide information, advice, and signposting to other agencies/services.
  • Contribute Social Work expertise to the wider Multi-Disciplinary Team/REaCH service.
  • Work with people in a person-centred way to maximize their strengths and assets.
  • Network with other professionals and engage in multi-agency working.
  • Work confidently with people living with risk, demonstrating flexibility, resilience, and creativity.
  • Support other colleagues/apprentices within a small social work team.
  • Attend team meetings and appropriate training.
  • Adhere to council policies and demonstrate knowledge of Adult Law.

Essential Requirements:

  • Qualified Social Worker with a relevant degree (DipSW, CQSW, BA/MA Social Work).
  • Registered with Social Work England.
  • Experience in working with adults and knowledge of relevant legislation, including the Care Act 2014.
  • Excellent assessment, communication, interpersonal, and analytical skills.
  • Ability to work independently and as part of a team.
  • Commitment to person-centred practice and promoting independence.
  • Enhanced DBS check.
