Support Service Co-ordinator (Young Parent Supported Housing Scheme)

Salary: £29,000 - £30,000 per annum
Location: Durham Region
Contract: Fixed Term until March 2025
Hours: 37 hours per week, including some weekends and on-call shifts

Overview:
We're seeking an experienced Support Service Co-ordinator to join a dedicated Care and Support team. This role centres on providing essential support to young families within a supportive housing environment, helping them towards independence. Based at a residential hub with nine flats and communal areas, this role is integral to building a thriving, supportive community.

Key Responsibilities:
  • Plan and run activities to engage residents, improve social connections, and support personal development.
  • Process new referrals, assess client suitability, and manage onboarding.
  • Work closely with residents to develop person-centred support plans and hold regular progress meetings.
  • Oversee property management, compliance, and health and safety at the site.
  • Attend multi-agency meetings, providing updates and written reports on cases.
  • Assist with income maximisation and help residents access welfare benefits.

Ideal Candidate:
You'll have experience working with vulnerable client groups, a strong understanding of adult and child safeguarding, and a supportive, non-judgemental approach. A valid driving licence and access to a vehicle are required for occasional outreach work across County Durham. The role requires flexibility, emotional resilience, and the ability to work independently when needed.
