Back to searchSocial Worker based in a hospital social work team covering discharges from acute hospitals and intermediate settings.  In this position you will need to be able to work in a fast paced environment and have recently worked as a qualified Adult Social Worker with experience in keeping and maintaining accurate records, making sure assessments and support planning is done in a timely manner to reduce delays within our hospital discharge pathways.  You will have excellent communication skills and feel comfortable to pick up new and existing cases in particular safeguarding and mental capacity assessments.
